If you manage multiple long-term conditions, our CCM program provides proactive care coordination, personalized care plans, regular check-ins, medication support, and continuity beyond office visits. 

From evaluation to treatment, our allergy and ENT services provide relief and long-term management for ear, nose, and throat conditions. For eligible patients, we also offer allergy testing and injection therapy to help reduce symptoms and improve quality of life.  Currently seeing patients on Thursdays for ENT/Allergy in Joplin.

 Our program is led by a board-certified Obesity Medicine physician, offering evidence-based strategies for safe, sustainable weight management. Through personalized care plans, lifestyle support, and medical expertise, we partner with you to achieve lasting health and wellness 

Our current qualifying patients can stay connected from home. With RPM, we can monitor vital health data—like blood pressure, pulse, glucose, or weight—in real time, allowing us to intervene early and help prevent complications.
